All referrals should comply with the Referral Standard guidelines and include in particular:
- History of condition including diagnosis
- Past treatments and response
Tests
- ENT consult required for patients presenting with voice disorder
- Paediatric workup required for children presenting with complex feeding issues

? Known or suspected dysphagia with aspiration pneumonia and/or weight loss
