Social Work in Your Community
We provide a welcoming, safe and confidential space for adults. We work with other Allied Health professionals to provide services and programs aimed at improving your health and quality of life.
We are a member of the Safe In Our Care network.
We can support you by providing:
- Confidential counselling/emotional support
- Information about and/or referrals to other community services
- Advocacy (helping people to have a voice to address their concerns)
- Therapy groups and educational programs around chronic condition self-management and general health and wellbeing
- Health coaching and helping people to manage their chronic conditions
We support people who are dealing with concerns such as:
- Depression and anxiety
- Loss and grief
- Loneliness and social isolation
- Access to services
- Life changes
- Elder abuse
- Emotional trauma recovery
- Chronic health conditions
- Illness, injury and disability
We also work as part of a broader team of professionals who respond to local community events that require recovery. Community social workers in rural settings also participate in community development.
This is a cost-free service.
Community social work - North
Groups/Classes
- Overcoming pain and living life (OPALL) – a ten week course for people experiencing complex persistent pain comorbid with one or more other conditions. Centrally sensitised pain conditions such as fibromyalgia, neuropathic pain and pain that can not be resolved by surgery
- Get the Most out of Life – a six week For anyone living with an ongoing chronic condition or who is caring for someone who is.
- Breathe – an eight week course For people who are living with chronic worry, anxiety or chronic stress
- Mindfulness in Motion – a six week course for people who want to explore how to use simple modified tai chi movements to increase motivation
- Men’s Anxiety Program
- Chronic Pain Group – a six week course for people living with chronic pain
- Creative groups (art, music and drumming)
- Specialist health condition programs

Community social work - North West
Based at the Devonport Community and Health Services Centre - The service covers the '64' North West region.
Our service role is divided into three main categories:
- Short term counselling and support
- Community development and
- Health promotion.
Alongside these roles, we are also involved with community recovery, which assists communities following accidents and natural disasters.
We provide information and referral services for people affected by adverse health outcomes.
Services requested often:
- Grief and loss support
- Coordination of care and services
- Stress management
- Elder abuse support
- General and personal support
- Group work
- Practical and financial support.
We work with local services to form partnerships to develop, run and/or support programmes.
Referrals
We accept referrals from any health practitioner or service provider, and we accept direct referrals from clients.

Community social work - South
Community Health Social Workers are available in community health centres and provide home visiting where required. Community Health Social Workers work in multidisciplinary teams offering support, counselling, arranging health promotion events and support for the community following accidents and natural disasters.
