What is Hospital@home
- Hospital@home is an interdisciplinary service in Southern Tasmania that provides hospital-level care within the comfort of your own home or community setting, if it is safe to do so.
- This service is for adult patients and provides care in your community residence, including Residential Aged Care Facilities (RACFs).
- You will be referred to Hospital@home by your health care professional.
- Hospital@home provides three services:
- Rapid Response – short-term intervention for people who are at risk of needing to go to hospital with an acute illness or injury.
- Acute Care – inpatient admitted care that would otherwise be provided in the hospital setting.
- Subacute Care – specialised interdisciplinary care to older patients who require Comprehensive Geriatric Assessment and management.
- Hospital@home is not an emergency service and does not replace the need to call 000 if your situation is critical and/or an urgent response is required.
How does it work?
- Any health care professional can refer you to Hospital@home if there has been a change in your health.
- A Hospital@home Registered Nurse will have a discussion with your referring health care professional to see if Hospital@home can safely meet your needs in the home and community environment.
- An initial assessment will be conducted by one of our Hospital@home clinicians. This may happen in your home or in our clinic environment. Our team will talk with you about your health needs and together a plan for your care will be developed.
- You will receive daily contact from one of our Hospital@home clinicians.
- If you are admitted to our acute or subacute service, you will be classed as an inpatient of the Royal Hobart Hospital and during your admission you will be unable to visit your usual General Practitioner for Medicare billing reasons.
- Your General Practitioner will receive a comprehensive clinical summary when you are discharged from our service.
Who will be involved in your care?
- Your healthcare team may include:
- Medical specialists
- Nurse practitioners
- Registered nurses
- Pharmacists, pharmacy technicians
- Allied health professionals (physiotherapist, occupational therapist, social worker, speech pathologist, dietitian, podiatrist, and psychologist).
How to contact us
- This service operates 24 hours, seven days a week (including public holidays).
- Home visits occur between 7:30am to 10:30pm, seven days per week (including public holidays).
- A registered nurse will be available via phone to seek advice and support 24 hours a day.
