- Regarding the NRWH 17yo patient who allegedly stabbed a doctor on April 9, please provide day-by-day breakdown of how many times his record/patient files were accessed inappropriately from April 9 until the last day his file was inappropriately accessed.
- Please provide the final report/results of the audit undertaken in early April 2023 on the inappropriate accessing of patient records by Hospitals North, South and North-West staff
- Since June 2, 2023 when Kathrine Morgan-Wicks formally directed all staff to not inappropriately access records, to current, how many times have records been accessed inappropriately? Please provide breakdown for Hospitals North, South, NW.
- In financial year 2022-23, how many times were records accessed inappropriately by Tasmanian Health Service staff? Please provide breakdown by hospital group
- Regarding employees who have inappropriately accessed records in the last five calendar years (since 2018), please provide a list of the top five employees (de-identified) by number of times records have been inappropriately accessed and the date range they were accessed, and what hospital group they are employed at – i.e. #1, Hospitals North, 17 inappropriate accesses between X and Y date

Decision and Statement of Reasons
At this stage the public authority has not finalised its audit undertaken in early April 2023 on the inappropriate accessing of patient records by Hospitals North, South and North-West staff. It is unknown when this audit will be finalised. However, upon finalisation, the applicant will be notified, and the release of the final report will be assessed in accordance with the Act.
